This is a Mabe Pearl. In the warm lagoons of Manihiki & Penrhyn in the Northern Cook Islands, the Framer
secures a dome shaped teardrop, heart etc on the inside of the shell (Black lip Mother of Pearl Pinctada argaritifera) The Oyster is returned to the sea for 12-18 months because the dome causes an irritation, the oyster responds by putting layers of Nacre over the shape, thereby creating this Pearl luster.
